Perhaps for this rosh hashana it will be enough to honestly come to a realization that your life is not manageable and you need to do something concrete to change it.
Then you can work on things next year.
I remember I once had a rebbe that said after succos, "Ok. In ellul we only had time to work on the four steps of teshuva. Now we have time to work on all the rest of the steps that rabbeinu yonah says are part of teshuva.

I wouldn't focus on next year.

Tell God that you would like to be sober for right now...that itself should lift your spirits.

Hashem is not a mean old tyrant.

He created you, and just like every inventor, He knows the specs of His creations very well.
He knows and understands everything about you - your potential, your shortcomings, your accomplishments - everything. He loves His creations.

Rosh Hashana is about accepting Hashem as king. It's when we commit to serving Him to the best of our abilities.

We need to put thoughts of this "struggle" out of mind on Rosh Hashana. Don't think about whether we are over it or not, whether we will ever give in again or not.

Leave the accounting to Hashem. He's much better at it.

Focus on committing to serve Hashem to the best of our abilities, and simply ask Hashem to help us make the right decisions, as they come up, moment by moment.
