Welcome My Friend.

Help is here - Stick around and pick it up.

You said "I cant ignore my situation anymore " You are a lucky person that you realized that your life cannot continue like this. In the 12 Steps that is the most fundamental Step realizing our lifes have become unmanageable.

Your courage to get up and do something about it is amazing.

My friend - do you have filters/accountability software on your computers?

Keep on posting.

Have a uplifting Shabbos.

You're not lost! You have a tremendous opportunity on your hands. Look back at your previous falls and see what were the triggers, and try to change a few things around. Lost implies not having a clear path and not being able to get where you want to go. In this case, you know where you want to end up, and you could look at the previous stumbling blocks and take a different path that you originally had in mind, a better path. (BTW I'm speaking to myself here just as much as I am speaking to you.) You're definitely on the right path. You may just have to tweak a few things. Good luck!!!

You are never lost.
Don’t forget your name. There is always, really always a 2nd chance.
There is a chassidish mamar from one of the Yesod Hoavoda (i think), If a person just killed somebody and still holds the bloddy knife in his hands and can not go and daven to hashem like nothing happened, than he does not know what a chossid means.
The only thing the YH wants from you is to be lost.
The Ateres Yohishua sais that the YH wants much more your yiush after the aveiroh than the aveiroh itself.
Have look at this movie from Aish http://www.aish.com/h/hh/yk/101461854.html

let's be lost together!

if we're lost together then it's not called 'lost' anymore it's called an 'adventure'

we'll find our way out of this together....

where are we right now, 2nd chance? in a forest? desert? confusion?
